Multiple Entry Visa for Nationals of Philippines

REQUIRED DOCUMENTS

  1. Valid passport
  2. Visa application form with 1 recent passport size photo https://www.mofa.go.jp/files/000124525.pdf
  3. Valid Work Permit/ PR Card* to Canada(must be valid until the date you return to Canada) *Confirmation of PR is NOT acceptable.
  4. A Letter explaining the purpose of multiple visits
  5. A photocopy of entry stamps to Japan within the last 3 years if applicable (old passport may be required)
  6. Employment Letter issued within the last 3 months
  7. T4 from previous year
  8. Flight itinerary(Not an actual ticket, a screenshot printout of your entire planned flight itinerary from Ontario) One-way ticket is NOT acceptable.
  9. Hotel booking for your entire stay(screenshot printout)
  10. Schedule of Stay https://www.mofa.go.jp/j_info/visit/visa/pdfs/application7.pdf
  11. Bank statement for the last 6 months(name, date and account balance with sufficient fund)
  12. Proof of kinship(marriage certificate, birth certificate etc.) for a family application
